If elected, I will represent and act in the best interest of HCAOA and its members on behalf of the home care industry, NOT on behalf of any one organization or special interest.

Committee Participation *

I am willing to serve as a committee member and/or chair of an HCAOA committee TBD.

Meeting Attendance *

I commit to attending at least three (3) of the four (4) HCAOA Board meetings in 2024, including the first meeting in January which is mandatory.
