​What 400+ Home Care Leaders Said About AI & Why It Matters

As an industry, we talk about AI constantly. But how much of that conversation is grounded in what providers are actually experiencing? AxisCare recently commissioned an independent survey to answer that question, and the results are worth unpacking.
1. AI adoption in home care is real, not hype.
Ninety-one percent of survey respondents said they are already using or planning to use AI for home care operations management. This isn't a trend on the horizon; it's happening right now, in agencies of every size, across every market.

2. The value is already showing up.
Among agencies actively using AI, 94% reported tangible benefits, with 64% citing cost reductions and 59% pointing to improved strategic decision-making. More than a third said AI helped increase revenue. These aren't projections; they're results from agencies that made the leap.

 3. More experience with AI leads to more confidence.
One of the most telling findings: confidence in AI grows in direct proportion to usage. Among agencies in the most advanced "transforming" stage, 93% said they were very confident in AI's value. The agencies most hesitant about AI are often the ones with the least exposure to it, which suggests that simply getting started may be the most important step. 

4. Caregiver staffing is the growth barrier AI can help solve.
Nearly all respondents (99.5%) said their agency will grow over the next five years, but 85% acknowledged growth will be constrained by market forces. Recruiting, retention and scheduling challenges topped the list. Critically, 91% of leaders identified AI as a tool that could directly address their specific growth barriers. That's a significant signal for how agencies should prioritize investment.

5. Human connection must remain non-negotiable.
For all the optimism, 57% of leaders expressed concern that AI could diminish the human connection with clients. This concern is valid and should shape how every agency approaches implementation. AI should reduce administrative burden so caregivers can focus on care, not replace the relationships that define this industry.

6. The barriers to AI are cultural, not financial.
Three-quarters of respondents said their agency faces barriers that are slowing AI adoption, but the obstacles at the top of the list may surprise you. The biggest hurdle isn't budget or technology; it's culture. Forty-one percent cited general concerns about the ethics and morals of AI, and 30% said employees are resistant to changing the way they work. Only 15% pointed to leadership disinterest in making investments. That tells me the industry has more willingness at the top than many assume, and that the real work of AI adoption is change management as much as anything else.

The data tells a story of an industry that is optimistic, clear-eyed about the challenges ahead, and increasingly willing to act. For agency leaders still on the fence, the best time to start is now.
